Once discharged, your GP can issue a Sick Note, based on your Hospital discharge letter.&nbsp;If you think this is your situation, our Reception Team will assist you.<\/p>\n\n\n\n<p><strong>Please note a Sick or Fit Note may be backdated <strong>if you have already consulted with the GP about this specific condition\/episode of illness<\/strong>.  Emergency or urgent appointments should not be used to request Sick \/ Fit notes.<\/strong><\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/www.brunelmedicalcentre.nhs.uk\/your-record\/medical-request-forms\/patient-request\/\">Patient Medical Request Form<\/a><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Self-Certificate<\/h2>\n\n\n\n<p>If you have been sick for less than seven days (this includes non-working days), you can self\u2013certify your illness using a SC2 form. You can obtain this form from your employer or the <a href=\"https:\/\/www.gov.uk\/government\/publications\/statutory-sick-pay-employees-statement-of-sickness-sc2\">HMRC website<\/a>. <\/p>\n\n\n\n<div class=\"wp-block-file\"><a id=\"wp-block-file--media-936f98a3-c7d0-4fed-b197-c8218880ed22\" href=\"https:\/\/www.brunelmedicalcentre.nhs.uk\/wp-content\/uploads\/2020\/03\/self_certification_certificate_1-1.pdf\">Self-certification certificate<\/a><a href=\"https:\/\/www.brunelmedicalcentre.nhs.uk\/wp-content\/uploads\/2020\/03\/self_certification_certificate_1-1.pdf\" class=\"wp-block-file__button wp-element-button\" download aria-describedby=\"wp-block-file--media-936f98a3-c7d0-4fed-b197-c8218880ed22\">Download<\/a><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">More than seven days off sick<\/h2>\n\n\n\n<p>If you&#8217;re off work sick for more than seven days your employer will normally ask for a Fit Note (or Statement of Fitness for Work) from your GP or Hospital Doctor.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How to count&nbsp;sick days<\/h2>\n\n\n\n<p>When you&nbsp;work out the number of days that you&#8217;ve been sick, you need to count all the days in a row you&#8217;ve been sick, including days you don&#8217;t normally work such as weekends and bank holidays.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Statement of Fitness for Work<\/h2>\n\n\n\n<p>The Doctor will provide you with a Statement of Fitness for Work (\u2018Fit Note\u2019) if you are still not well enough to work.
