class=\"is-style-checkbox\"><li><strong>Photographic Proof of Identification<\/strong> (passport, driving license or birth certificate)<\/li><li><strong>Proof of Address<\/strong> (bank statement or utility bill no more than 3 months old)<\/li><\/ol>\n\n\n\n<p><strong>IMPORTANT:<\/strong>&nbsp;Please deliver to the practice copies of&nbsp;<strong>two forms of your identity<\/strong>&nbsp;from the above list \u2013 one form of identity must contain a photo of yourself and the other must provide evidence of your current address.<\/p>\n\n\n\n<p><strong>You can upload copies of your identity with the form below.<\/strong>&nbsp;If you prefer to submit your documents in person please speak to the reception team.<\/p>\n\n\n\n<p><strong>Please allow 7 days before your registration will be complete
