Please note, our system uses Smart Form technology. It is therefore important that all mandatory questions are answered in order to ensure progression of the form. Failure to do so or to provide comprehensive information may result in delays with the application. If at any point you do not meet the eligible criteria for the particular request, the system will signpost you to the next most appropriate pathway for further assistance. The team endeavour to process all forms within 48 working hours, however, please allow up to 5 working days for us to review the submitted form and respond. We will communicate with you about the outcome directly to your email address. Please note that it would not be expected that the clinician would call patients to discuss the issuing of certificates, letters, forms or fit notes. Kindly ensure that your contact details are correct and up-to-date.
